You may submit the form online or bring it to the Taxi Service in person.
You will need the following information to submit online:
- An electronic copy of your Driver's License or ID to be uploaded
- Please download and complete the Purchased Medallion Beneficiary Form here. Fill out the beneficiary form to be uploaded
